Billing and Insurance

OPTM Sports & Physical Therapy, Inc. will bill your private, auto or workers compensation insurance carrier.

Billing services are a courtesy provided for all major medical plans. OPTM does not bill secondary insurance companies or third party carriers. As a courtesy, OPTM will bill supplemental plans for all Medicare patients.

Billing your insurance company is a courtesy provided by OPTM. We understand that special circumstances may occur. Prior to your first appointment, please contact our billing department to discuss these or any other insurance issues.

We may not be an in network provider for your insurance company, but that does not mean that your insurance company will not pay for treatment at OPTM. The coverage will be different for an out-of-network provider. Please call your insurance company or our office to find out your specific coverage.
