Review Policies
Prepare for your first visit
  • Bring your insurance card. If you have Medicare, please bring your secondary or supplemental insurance card as well
  • Bring your signed referral for physical therapy that was given to you by the referring physician
  • If we are treating an area from the waist up, please wear a tank top and exercise bottoms
  • If we are treating an area from the waist down, please wear shorts or loose pants that can be rolled up above your knee and a T-shirt
  • Please wear or bring athletic shoes
  • Each clinic provides a changing area
  • If patient is a minor, a parent or legal guardian must accompany the patient
What to expect at your first visit
  • If you completed online registration you will be asked to review your submitted forms
  • If you have not completed online registration, arrive 15 minutes early to complete these forms
  • You will review the benefits given to us by your insurance company and sign the Financial Policy
  • A payment towards your co-pay, co-insurance, and/or deductible will be collected. We accept MC, VISA, personal check or cash
  • Your therapist will review the patient questionnaire with you and complete a physical therapy evaluation
  • Physical therapy treatment will be provided at the initial evaluation so please be prepared with the appropriate clothing
  • Future appointments will be scheduled based on your treatment plan
